The Simpler Form

In my quest to gather as much data as possible, I wanted to make things as simple as possible for anyone willing to assist me. So, with this in mind I thought I would create a really simple and straight forward version of the data capture form that I previous shared to gather data.

Firstly, this one doesn’t require your name and email, should you wish to remain anonymous in this attempt to gather information on paranormal activity. However remaining anonymous does mean I will be unable to contact you (should I wish to) in order to gather further details on your experience.

Simply enter the location name and county in the ‘Location of Activity’ field, then enter the date of the day or night the activity occurred on. Then go ahead and check the box next to the type of activity that occurred so we can categorise it. As sometime we can experience multiple types of activity at once, please feel free to check more than one box.
